Job Responsibilities:
- Conduct comprehensive assessments of patients’ physical, emotional, and social needs.
- Develop, implement, and monitor individualized care plans based on assessment findings.
- Coordinate care among different providers, ensuring continuity and efficiency of services.
- Facilitate patient education on managing health conditions and navigating healthcare systems.
- Advocate for patients and families to connect them with required resources and services.
- Monitor patient progress, adjusting care plans as necessary and documenting changes.
- Collaborate with healthcare providers to optimize treatment and enhance patient outcomes.
- Analyze data and patient trends to identify opportunities for improvement in care delivery.
- Ensure compliance with healthcare regulations, organizational policies, and quality standards.
- Participate in case reviews, clinical team meetings, and professional development activities.
- Provide guidance and support to healthcare staff in implementing care management interventions.
- Utilize healthcare technology and electronic health records to streamline processes and documentation.
